Abstract:
The medical field of treating chronic pain is still in its infancy. It was only in the late 1980s that leading physicians trained in treating the chronic pain of terminally ill cancer patients began to recommend that the "opioid therapy" (treatment involving narcotics related to opium) used on their patients also be used for patients suffering from nonterminal conditions. The new therapies proved successful, and prescription pain medications saw a huge leap in sales throughout the 1990s. But opioid therapy has always been controversial. The habit-forming nature of some prescription pain medications made many physicians, medical boards, and law enforcement officials wary of their use in treating acute pain in nonterminal patients. Consequently, many physicians and pain specialists have shied away from opioid treatment, causing millions of Americans to suffer from chronic pain even as therapies were available to treat it.

Almost everyone agrees that the U.S. health care system is in dire need of reform. But there are differing opinions on what kind of reform would be best. Some on the political left would like to see us copy one of the government-run “single-payer”systems that exist in Western Europe, Canada, and New Zealand, among other places. Proponents of socialized medicine point to other countries as examples of health care systems that are superior to our own. They insist that government will make health care available on the basis of need rather than ability to pay. The rich and poor will have equal access to care. And more serious medical needs will be given priority over less serious needs.

The legal definition of GENOCIDE: The international legal definition of the crime of genocide is found in Articles II and III of the 1948 Convention on the Prevention and Punishment of Genocide. Article II describes the two elements that constitute the crime of genocide: The mental element , meaning the " intent to destroy, in whole or in part, a national, ethnic, racial or religious group, as such", and The physical element , which includes five types of violence described in sections [a] though [e] as follows: [a] Killing members of the group; [b] Causing serious bodily or mental harm to members of the group; [c] Deliberately inflicting on the group conditions of life calculated to bring about its physical destruction in whole or in part; [d] Imposing measures intended to prevent births within the group; [e] Forcibly transferring children of the group to another group.

364. Africa Action Talking Points on North-South Peace Agreement in Sudan

On Sunday, January 9, African leaders and world diplomats will gather in Nairobi, Kenya to witness the signing of an historic peace deal intended to end Africa's longest-running civil war. This conflict between the Sudanese government in the north and the Sudan People's Liberation Army /Movement (SPLA/M) in southern Sudan has raged for m o re than two decades. Sunday's signing ceremony marks the culmination of two years of form al peace talks and many years of periodic negotiations, sustained by regional and international diplomacy. The signing of this peace deal could mark an historic moment for Sudan, by bringing to an end decades of violence and devastation in Africa's largest country. It could similarly mark an important moment for the entire African continent. However, this peace agreement does not cover the ongoing conflict in Darfur, western Sudan, where the Sudanese government continues to wage a campaign of genocide against civilians from three ethnic groups. Over the past two years, up to 400,000 people have died, and 2 million more have been made homeless in Darfur as a consequence resulting in what the UN has called the world's worst humanitarian crisis.

With the 4th World Water Forum scheduled for Mexico City in March 2006, the Aspen Institute and the Nicholas Institute for Environmental Policy Solutions at Duke University conducted a multistakeholder dialogue to help highlight the importance of global water issues, suggest steps to provide services more rapidly and effectively, and to identify and draw attention to constructive ways the US government and other US participants can take part in the Forum.

Abstract:
A stronger focus on jobs and growth is part of an effort to renew the Lisbon strategy. Yet the view that economic expansion contributes to maintaining Lisbon's other goals of social cohesion as well as the environment is somewhat optimistic. First, there are structural trade-offs among the central elements of the Lisbon strategy. Escaping these trade-offs temporarily is sometimes possible but requires policy changes. Second, higher productivity (growth) may not provide more structural room for governments to manoeuvre. It leads to higher tax receipts but also to higher public expenditures since public sector wages and social security benefits are linked to productivity. In contrast, more employment (jobs) is associated with a smaller public sector. But to engineer the increase in employment, changes in welfare state arrangements are needed. In other words, focussing solely on the sick child will probably harm the other children.

367. Multilateral Market-Access Reforms of the Doha Round: A Preliminary Assessment of Implications for EU Agricultural Trade

Abstract:
The July package of the Doha Round of trade negotiations stipulates that a tiered-formula approach should be used to significantly reduce market access barriers across countries, implying that the EU would have to make larger cuts to its high external tariffs, in comparison with many other WTO members such as the US. This paper provides a preliminary assessment of the likely impact of the tiered-formula reform approach on EU agricultural sectors. Numerical simulations of a multilateral market-access reform scenario show that such cuts would lead to across-the-board decreases in intra-EU trade flows, as compared with a baseline projection. While intra-EU trade flows would decrease, the EU's trade with the rest of the world would increase. Yet such increases would not be symmetric – imports into the EU would increase more than exports, resulting in larger external trade deficits or smaller external trade surpluses in many EU agricultural products. Further, the resulting adjustments in member states' production and net trade positions are not equal: the new member states would generally lose part of their export shares in the EU market to external competitors, as highlighted in the cases of bovine meat and dairy products.

Cet article analyse le débat actuel sur le dilemme humanitaire et la crise de l'action humanitaire. Il met en question la diagnose et les conclusions dominantes. L'article présente l'ambiguïté comme un phénomène structurel de l'action humanitaire. L'analyse des cas concrets du dilemme humanitaire, notamment de la Corée du Nord, révèle différentes catégories d'organisations humanitaires. L'auteur conclut que l'humanitarisme est caractérisé par un état de confusion par rapport aux principes les plus fondamentaux de l'action humanitaire. Ainsi, les organisations d'aide font face à une crise de cohérence.

369. The Sources of Stigma for Means-Tested Government Programs

Stigma has been shown to discourage participation in means-tested government programs. Prior research cannot explain why this deterrent effect varies in intensity across different individuals and programs. We develop a more comprehensive model of the possible determinants of stigma associated with means-tested programs than has previously been suggested by studies of welfare stigma. We test hypotheses using unique data based on interviews with 1405 respondents in ten states and the District of Columbia. The results suggest that there are two distinct forms of stigma related to participation in means-tested government programs: one related to self-identity, the other to threats of negative treatment. Both forms of stigma are more pronounced for welfare compared to Medicaid. The sources of stigma identified by conventional treatments of welfare stigma (e.g. individual attributions of responsibility for poverty) have significant explanatory power, but neglect other important influences. We find that stigma is exacerbated by poor health and by minority status. Stigma is also fostered by the ways in which means-tested programs are implemented, including negative interactions with case workers, long waiting times and for Medicaid, applications for benefits in alternative enrollment sites such as health centers or hospitals. These findings suggest new points of leverage for addressing the potentially deleterious consequences of stigma.

Jammu and Kashmir (J) has been a continual bone of contention, the object of three wars and a theater of engagement in a fourth war, between the two countries. Since 1989, insurgency has consumed Indian-administered Jammu and Kashmir (IJK), claiming at least 45,000 lives. All major attempts at resolution by the international community have failed, including those through the United Nations. With overt "nuclearization" by India and Pakistan in 1998, and with "jihadist" militants playing an increasingly prominent role in the insurgency since the mid-1990s, the Kashmir conflict also bears the marks of a distinctly twenty-first century security predicament.
