11. Maximizing the Impact of Aid to Pakistan: Leverage Reform and Local Capacity

Abstract:
The effectiveness of U.S. civilian assistance to Pakistan depends on sustained funding commitments from the United States and sustained commitment to economic and institutional reform from Pakistan. Weak public institutions and poor governance have greatly impeded Pakistan's development. U.S. assistance should focus on strengthening institutions systemically. Direct assistance to the Pakistani government—through financing that supports specific reform programs and policy initiatives and cash-on-delivery mechanisms that offer assistance after agreed performance criteria are met—could incentivize Pakistani public institutions to improve service delivery. Pakistan's devolution of authority to the provinces offers an opportunity for well-targeted and cost-effective initiatives to incentivize improvements in provincial public service delivery in such areas as basic education, health and policing.

With the rise of the Islamic State of Iraq and al-Sham (ISIS) and the explosion of violent conflicts from Tripoli to Gaza, the Middle East is looking more unstable and unpredictable than ever. While the focus in Washington is centered on jihadist extremists in Iraq and Syria at present, the threat from al-Qaeda in the Arabian Peninsula (AQAP) against the United States continues. Top al-Qaeda leadership in Yemen is hailing the territorial gains of ISIS in Iraq, and some al-Qaeda operatives are imitating ISIS' techniques such as public slaughters of those deemed infidels, prompting fears of cooperation between two of the most active Islamist militant networks. Recent aggression by the Houthi movement, a Zaydi Shia rebel militia, against state institutions and tribal opponents has opened a new front of instability and security vacuum that AQAP is all too ready to exploit. Inattention to the interconnected nature of tribal conflict, terrorist activity, poor governance, economic grievances and citizen discontent is proving to be a dangerous combination for both Yemen and the United States. The Yemeni context may seem far from the current focus on Baghdad and Damascus, but getting the US strategy right in Yemen will have consequences for regional stability and core US interests throughout the Arabian Peninsula and beyond.

13. Improving Maternal, Neonatal, and Child Health in Ghana

In June 2014, a small team from the CSIS Global Health Policy Center traveled to Ghana to examine U.S. bilateral support for maternal, neonatal, and child health (MNCH). The purpose of the trip was to plan a return visit with a delegation of U.S. congressional staff in August 2014. Ghana's mixed progress toward meeting Millennium Development Goals (MDG) 4 and 5 related to maternal and child health; its strong relationship on immunizations with Gavi, the Vaccine Alliance; and its longstanding partnership on health with the United States were all reasons we decided to examine the country's MNCH situation. By late July, the acceleration of the Ebola outbreak in West Africa led us to postpone the trip until emergency preparations are not a major focus of the Ghanaian government, the United States, and other partners. Considering the fruitful meetings we had in June, we have captured here some of our initial impressions, observations, and recommendations.

14. Is Anyone Listening? Does US Foreign Assistance Target People's Top Priorities?

The United States government has made repeated declarations over the last decade to align its assistance programs behind developing countries' priorities. By utilizing public attitude surveys for 42 African and Latin American countries, this paper examines how well the US has implemented this guiding principle. Building upon the Quality of Official Development Assistance Assessment (QuODA) approach, I identify what people cite most frequently as the 'most pressing problems' facing their nations and then measure the percentage of US assistance commitments that are directed towards addressing them. By focusing on public surveys over time, this analysis attempts to provide a more nuanced and targeted examination of whether US portfolios are addressing what people care the most about. As reference points, I compare US alignment trends with the two regional multilateral development banks (MDBs) – the African Development Bank and the Inter-American Development Bank. Overall, this analysis suggests that US assistance may be only modestly aligned with what people in Sub-Saharan Africa and Latin America cite as their nation's most pressing problems. By comparison, the African Development Bank – which is majority-led by regional member nations – performs significantly better than the United States. Like the United States, however, the Inter-American Development Bank demonstrates a low relative level of support for people's top concerns.

15. Combating Global Poverty: Investing in the Governance and Growth Nexus

Foreign aid donors face a changed development landscape that necessitates a new approach to programming resources. In the last 20 years, countries across the developing world demo cratized, began to improve their governance, and experienced substantial economic growth. Yet, significant challenges remain that must be tackled, many of which fall within the governance and growth nexus. These issues—government effectiveness, rule of law, regulatory policies related to the business and investment climate, and barriers to entry to the formal economy—are the preeminent challenges to expanding broad- based economic growth and continuing to reduce global poverty. The United States needs to shift its focus away from meeting basic human needs toward broader institutional development if it is to increase support for the governance and growth nexus. U.S. foreign aid is overwhelmingly directed toward global health and the delivery of other public goods. This must change.

16. U.S. Government Funding for Media: Trends and Strategies

Abstract:
U.S. government support for international media development is declining. Spending by the Department of State and USAID for media freedom and freedom of information programs has dropped 43.5 percent in the past five years–from $135 million in Fiscal Year 2008 to approximately $76.3 million in FY 2012, according to figures supplied by the State Department. This trend and others related to U.S. government support for media development are the subject of this report, which analyzes how U.S. government grant making for media assistance has changed over time and looks at possible reasons those changes have occurred. The data underpinning the report is drawn from two main sources: the State Department's Office of Foreign Assistance Resources, which tracks allocations for foreign aid, and from an in-depth examination and analysis of grant proposals solicited by the U.S. government over the past five years containing at least one media development component.

17. Replenishing the Global Fund in 2013

Abstract:
Hopes are high for a successful outcome of the 2013 replenishment process, through which the Global Fund to Fight AIDS, Tuberculosis and Malaria seeks pledges of $15 billion to support planned activities for 2014-2016. Yet at a time when some experts argue it is finally possible to “turn the tide” on HIV/AIDS, malaria, and tuberculosis, it is unclear whether countries and other donors will pledge adequate funding to meet the revitalized Fund's replenishment goal. The United States has an essential leadership role to play in helping ensure a successful 2013 replenishment outcome. In the short term it must come through with a strong pledge of its own and engage diplomatically with a range of donor and beneficiary countries to demonstrate support for the organization. In the longer term the U.S. will need to work with the Fund and other governments to identify new ways for public and private-sector entities, as well as individuals, to support the Global Fund's work.

U.S. policymakers and private-sector partners increasingly appreciate the importance of targeted U.S. investments in women's health to achieve global health outcomes, especially in sub-Saharan Africa.With budgetary constraints worsening, progress in women's health will require maximizing investments by engaging new partners, identifying program synergies, and aligning with countries' national priorities to meet women's needs. Such strategic coordination—involving maternal newborn and child health, voluntary family planning, and HIV and AIDS services—presents new opportunities to expand the impact of U.S. investments.
