Over the last decade, tremendous progress has been made in the prevention, care and treatment of HIV/AIDS, TB and malaria globally. The international community has played a key role in this progress and remains committed to the fight, but as implementing countries' economies grow, they are progressively graduating from international support. This could leave national governments, especially health ministers, uncertain about the future of financing available for their national health programs. Without sufficient resources from both domestic and international resources, there is a risk of resurgence of these diseases. If these trends continue, there may not be a "grand convergence" in health by 2035, resulting in enormous economic and social costs.

World Bank Group President Jim Yong Kim has publicly stated that achieving universal health coverage (UHC) and equity in health are central to reaching the two new overarching World Bank Group goals to end extreme poverty by 2030 and boost shared prosperity. Jim Kim has also rightly emphasized the need to close the gap in access to quality health services for the poorest 40 percent of the population and to eliminate point-of-service payments that impoverish people in every country.

In June 2014, a small team from the CSIS Global Health Policy Center traveled to Ghana to examine U.S. bilateral support for maternal, neonatal, and child health (MNCH). The purpose of the trip was to plan a return visit with a delegation of U.S. congressional staff in August 2014. Ghana's mixed progress toward meeting Millennium Development Goals (MDG) 4 and 5 related to maternal and child health; its strong relationship on immunizations with Gavi, the Vaccine Alliance; and its longstanding partnership on health with the United States were all reasons we decided to examine the country's MNCH situation. By late July, the acceleration of the Ebola outbreak in West Africa led us to postpone the trip until emergency preparations are not a major focus of the Ghanaian government, the United States, and other partners. Considering the fruitful meetings we had in June, we have captured here some of our initial impressions, observations, and recommendations.

U.S. policymakers and private-sector partners increasingly appreciate the importance of targeted U.S. investments in women's health to achieve global health outcomes, especially in sub-Saharan Africa.With budgetary constraints worsening, progress in women's health will require maximizing investments by engaging new partners, identifying program synergies, and aligning with countries' national priorities to meet women's needs. Such strategic coordination—involving maternal newborn and child health, voluntary family planning, and HIV and AIDS services—presents new opportunities to expand the impact of U.S. investments.

South Africa has the highest burden of HIV/AIDS in the world, with 5.6 million people living with the virus and over 400,000 newly infected annually. Since 2004, the U.S. government has committed more than $4 billion to combating HIV/AIDS in South Africa—the largest U.S. investment in HIV/AIDS worldwide. Continued progress in controlling HIV/AIDS in South Africa, the epicenter of the pandemic, is pivotal to sustained progress against the disease worldwide.

As the Millennium Development Goals (MDGs) target date of 2015 approaches, there is a growing sense of urgency among international agencies to intensify efforts on the global challenge of maternal health, where, according to the 2012 MDG progress report, levels of maternal mortality remain “far from the 2015 target”. In 2012, both the G - 8 and the African Union made maternal and child health a keystone of their respective annual summits , and the United Nations launched the Global Strategy for Women ' s and Children ' s Health at a special General Assembly event. A 2 012 global summit in London, co - led by the Gates Foundation, the UK government, and the UN Population Fund, generated $2.6 billion in donor pledges for family planning, a critical element of maternal health. The United States has made maternal health an increasingly important element in U.S. global health efforts, manifested most recently with the launch in June 2012 of the Saving Mothers, Giving Life initiative. The initiative, an ambitious public - private partnership intended “ to drive efficiencies, spur innovation, and ensure impact ” in maternal health , has the strong backing of Secretary of State Hillary Clinton, for whom maternal and child health, and women's empowerment more generally, have been consistent priorities.

The nine articles in this volume were chosen from over 150 submissions through a blind-selection process in Princeton. After individually reading a large number of submissions, attending editors first voted for their favorite papers via secret ballot, and we then facilitated an open debate to winnow down these choices to the cream of the crop. In the intervening months before publication, editors in Princeton from relevant policy areas worked with the authors to fact-check their work and fine-tune their arguments. While their geographic foci and policy subjects are multifarious, all the authors in this edition share a passion for changing the world through better policy. Whether it is women’s health issues in Tanzania, banking regulation in the European Union, diplomacy with Iran over its nuclear ambitions, or immigration policy in the United States, the authors implore global policymakers to change course in order to remedy contemporary problems in public affairs. This dedication to making the world a better place through public policy, in addition to demonstrating first-rate scholarship, make us proud to showcase these articles.

Since the 2000s, African cities have witnessed a series of interventions to improve water and sanitation. This policy brief outlines key lessons learned from the intervention experience, drawing on the UNU research project Multi-level Urban Governance for Total Sanitation (2011-2013) under the Education for Sustainable Development in Africa (ESDA) Project. It highlights the importance of multi-actor approaches for promoting: (1) an institutional framework to coordinate civil society organizations, community-based organizations, and the state agencies across levels; (2) policy recognition of water and sanitation as socially embedded infrastructure with gendered dimensions; and (3) the relevance of scientific research and university education to ongoing policy interventions.

50. Improving the Future of Maternal and Child Health in Sub-Saharan Africa by Investing in Adolescent Girls

The premise of this policy brief is simple: The most effective way to dramatically reduce maternal and child mortality in sub-Saharan Africa over the next decade is to invest in adolescent girls’ educational and economic opportunities today. The goals set forth by MDG 4 (decrease child mortality by two thirds) and MDG 5 (reduce maternal mortality ratio by 75%) are already unlikely to be reached by 2015. What is more, we will continue to fall short of these objectives over the next ten years if the developmental needs of adolescent girls continue to be neglected. Continued investment of resources to improve access to high quality health services, medical treatments, and technology development are necessary and important to reach these goals. But they are not sufficient as long as the root causes of poor maternal and child health outcomes remain unaddressed. Devoting resources to improving the social and economic opportunities of adolescent girls offers a proactive approach with multiple long-lasting effects on not only these girls directly, but also on their future families and communities. The profound effect of women’s education on child survival is well-established. Improvements in women’s education account for half of the decline in child mortality since 1970. Other equally impressive effects are less well-known. Girls who attend secondary school, for example, delay sexual debut, pregnancy, and marriage by several years, resulting in lower rates of sexually transmitted infections (STIs) and HIV, teen pregnancy, unsafe abortions, and single adolescent motherhood. Consequently, increasing girls’ schooling will make their pregnancy and motherhood experiences safer and reduce maternal mortality. Alleviating adolescents’ poverty and developing their income-generating skills will also offer girls alternatives to early motherhood and marriage and provide them and their families with the economic resources to improve their nutrition, sanitation, and utilization of health services.
