621. Millennium Development Goal 7: An Assessment of Progress With Respect to Water and Sanitation: Legacy, Synergy, Complacency or Policy?

Abstract:
Access to water and sanitation (target 10) is an important ingredient of quality of life. As per WHO-UNICEF assessments, globally, 77 per cent of population had access to water in 1990. This proportion has increase d to 83 per cent in 2002, thus, on track to achieve the target of halving the proportion of population without safe access by 2015. However, there is considerable regional disparity in progress which remains significantly low in many countries in sub- Saharan Africa. Also, the question remains whether increased access is same as sustainable access. In 2002, some 2.6 billion people worldwide did not have access to safe sanitation options. Of these, nearly 2 billion were in the rural areas. While in almost all countries, the proportion of people having access to improved sanitation in 2002 has increased compared to the status in 1990, in 27 countries including India, Ne pal, Lao PDR, Namibia, Ethiopia, Eritrea, and Yemen, two out of three people did not have access to improved sanitation in 2002.

Abstract:
Providing all beneficial care to those who need it is rapidly becoming unaffordable, even for a nation as rich as the United States. The highly decentralized U.S. payment system is unique in its lack of effective levers for limiting health care spending, and managed care has largely been ineffective. A different solution, considered extreme by many in the United States, is rationing. The need to ration healthcare has long been a reality in the United Kingdom where healthcare spending must be covered by an annual budget accounting for only 7.6 percent of GDP—about half the U.S. share. These decisions are perhaps most difficult in regards to treatment of conditions that are literally matters of life or death, such as coronary artery disease. This brief examines reasons for the differences in treatment and outcomes in the United States and Britain, and discusses the difficulty of rationing care in the United States, where a unique payment system now uses income from those with health insurance to cover the medical costs of the uninsured.

Abstract:
By using a game theoretical approach and the notion of “forum” or “regime shifting” this paper examines the multiple interfaces of the multinational pharmaceutical corporations (MNPCs) with their major counterparts at home and abroad, including the governments of the North and South, the international organizations and increasingly with national and international NGOs in the context of the spreading HIV/AIDS crisis. It shows how the execution of the MNPCs' strategies led to significant reactions from other actors, which led to further moves and countermoves in a strategic battle between representatives of the North and the South in the global health arena.

Abstract:
This paper analyses the impact of new institutional structures in global health governance on the realization of social rights in poor countries. Meanwhile, health is broadly seen as an import precondition for social and economic development. This leads to an integration of the “diseases of the poor” (basically infectious diseases) into strategies of fighting poverty. Considering the example of global HIV/AIDS politics, the paper argues that new governance modes increase the participation of civil society groups and affected communities, but that they are also frequently instrumentalised by powerful actors to pursue their particular interests. In fact, increasing resources are mobilized for the fight against poverty related diseases. The paper concludes that global health governance is characterized by a combination of moral values and material interests which does not guarantee a comprehensive realization of social rights, but which allows some progress in the fight against poverty-related diseases – a precondition of the possible further realization of social rights.

The United States spends more on health care than any other nation. In 2003, medical spending made up more than 15 percent of U.S. GDP, and if historical trends persist, this share will climb to more than one-third of GDP by 2040. With medical technology advancing at an ever-increasing rate, the potential for spending on procedures not worth their costs is growing. But there are few good ideas for reining in medical costs without hurting patients.

Abstract:
It is important to clarify the security dimensions of the HIV/AIDS pandemic because actions taken to confront the disease as matters of domestic policy or foreign aid may differ markedly from those taken to address threats to national security. Understanding the impact HIV is now having, much less forecasting its toll and effects twenty years hence, is difficult. Little scrupulous analysis of the political, military, economic, and general security effects of the pandemic has been performed, both because the area is poorly funded and the problem is extremely complex.
