401. Tariffs, Corruption and Other Impediments to Medicinal Access in Developing Countries

Abstract:
There are many factors which hamper health care delivery in the developing world. These factors include tariffs, taxes, corruption, such as bribes and other local price inflators on medicines and medical products. Non-tariff barriers, such as lengthy registration periods for medicines and onerous requirements to clear customs, also restrict the availability of medication in the developing world. According to the World Health Organization, approximately one-third of the world's population lacks access to essential medicine and proper medical treatment. Drawing upon extensive evidence from surveys and accounts from the field, this paper examines the impact of tariffs, taxes and other markups on imported medicines and medical products provided to lesser developed countries by pharmaceutical companies, not-for-profit groups, for-profit corporations, multilateral and bilateral aid and health agencies. The paper discusses how these regulatory barriers affect access to medication. The authors conclude that although efforts to reform the current system of government revenue generation through tariffs collection may meet resistance in many developing countries, especially those featuring systemic corruption and those with domestic production, governments which take steps to eliminate tariffs could in fact expedite health care delivery and consequently improve the well-being of their people.

The international aspects of corruption have received considerable attention in both research and policy: What determines whether a country is highly corrupt or not? Most research has sought to answer this question by considering each country as reflecting the same kind of mechanism explaining both the high and low outcomes. In this paper some of the theoretical explanations suggested in the literature are reviewed at the same time as it suggests how the question needs to be rephrased if each country's corruption rate is influenced by an internationally open economic system.

In most countries within South Eastern Europe a combination of past conflict and weak, but developing state institutions has resulted in varying degrees of control over the possession, use and trade in weapons in the first few years of the decade. Limited police performance, corruption and the growth of organised crime and trafficking networks, have effectively served to both justify illicit weapons possession in the minds of many, and also allowed for undesirable transfers and possession of illegal Small Arms and Light Weapons (SALW). Problems with co-operation and exchange of information, either at the national level between the different relevant departments or agencies, or internationally between governments, have also been evident.

407. Fuelling the Niger Delta Crisis

Abstract:
Less than a year before Nigeria holds its third national elections since the end of military rule in 1999, tensions are running high in the southern Niger Delta. A number of militant groups have begun allying themselves to local politicians with electoral aspirations. These groups and others continue to use legitimate grievances, such as poverty, environmental destruction and government corruption, to justify increasingly damaging attacks against government and oil industry targets. Removing the incentives for violence will require granting a degree of resource control to local communities. Engaging Delta groups in sustained, transparent dialogue also remains critical to finding a solution to the militant puzzle. Equally important, credible development efforts must be supported and stiff penalties for corruption imposed upon those who embezzle and squander funds.

On April 7, the State Department announced its plan for restructuring aid to the Palestinians in response to the formation of a government led by Hamas, which has refused Quartet demands to recognize Israel, cease violence and terror, and accept past diplomatic agreements. In order to target assistance toward the Palestinian people rather than the Hamas leadership, the United States will now provide the vast majority of its aid (some $203 million) for humanitarian needs, including food, health, and education programs primarily administered by United Nations agencies such as the UN Relief and Works Agency (UNRWA), the UN Children's Fund (UNICEF), and the UN World Food Program. An additional $42 million is allocated for "securing and expanding democracy," in an effort that "protects and promotes moderation and democratic alternatives to Hamas." Assisting the development of such a peaceful and democratic alternative -- as distinct from an immediate overthrow of Hamas -- will require the United States to support programs driven internally by Palestinians that can foster a broad-based political movement. Bolstering a centralized Fatah-like organization run by elites will only lead to further corruption and the continued alienation of the Palestinian public.

Economic inequality provides a fertile breeding ground for corruption and, in turn, leads to further inequalities. Most corruption models focus on the institutional determinants of government dishonesty. However, such accounts are problematic. Corruption is remarkably sticky over time. There is a very powerful correlation between crossnational measures corruption in 1980 and in 2004. In contrast, measures of democracy such as the Freedom House scores are not so strongly correlated over time, and changes in corruption are unrelated to changes in institutional design. On the other hand, inequality and trust-like corruption are also sticky over time. The connection between inequality and the quality of government is not necessarily so simple. The aggregate relationships between inequality and corruption are not strong. The path from inequality to corruption may be indirect, through generalized trust, but the connection is key to understanding why some societies are more corrupt than others. This study estimates a simultaneous equation model of trust, corruption, perceptions of inequality, confidence in government, and demands for redistribution in Romania, and shows that perceptions of rising inequality and corruption lead to lower levels of trust and demands for redistribution.

Kenya's independence leader, Jomo Kenyatta, of the Kenya African National Union (KANU), held power from independence in June 1963 to the time of his death in August 1978. He was succeeded by then Vice President Daniel arap Moi, who retained the presidency through Kenya's multiparty elections in 1992 and 1997. However, both elections were marred by controversy owing to political violence, widespread voting irregularities, and fraud.
