21. Covid-19 prompts the EU and the Netherlands to rethink global health

Abstract:
The Covid-19 pandemic prompted a strong re-engagement in global health. Because the pandemic coincided with geopolitical rifts between the US and China, but required a global response, the EU and its member states took responsibility to safeguard the World Health Organization (WHO) and initiated global arrangements for vaccine sharing for developing countries. Within the EU, mandates and global health functions have traditionally been underdeveloped and divided between the development and health sectors. For the Netherlands, this is perhaps the case to an even larger extent. Development funding has focused primarily on sexual and reproductive health and rights, and the health ministry has had limited interest and capacity on international health issues, with the exception of certain specific issues such as Anti-Microbial Resistance (AMR) and medicine prices. Spending on global health by both the EU and the Netherlands has been fragmented, with mainly ad hoc budgets being made available for the international pandemic response. This policy brief calls for a structural response and more coherent outlook on global health.

Abstract:
The coronavirus pandemic has spread throughout the world and Latin America has not been exempt from its health, economic and social impacts. The economic shutdown, as a result of a combination of stringent measures (self-quarantines, mandatory lockdowns, limited capacity in shops, factories and offices, border closures, etc.), is having a profound economic and social impact. In the labor market it has shocked both supply and demand. Within households, it has resulted in an increase in the unpaid workload, burdening women disproportionately, further reducing the time they can allocate to productive activities. The crisis’s impacts and depth are felt differently by they can widen existing gender gaps. In this paper, the authors explore the impacts of the crisis on employment in sixteen countries in the region. In addition, they analize gender impacts through four lenses: young people, people living in poverty, people living in rural areas and heads of the family. Researchers present a set of policy option aimed at integrating the gender approach into the entire pandemic world. Emphasizing the need for cross-sectional solutions, the authors propose policies in three main areas: the home, work and the spaces between work and home. This will enable socio-economic recovery policies to not only soften short-term impacts, but also further equal opportunities for women and men in the medium and long terms.

Abstract:
The entire world today is, in one way or another, affected by the spread of the novel coronavirus infection which the World Health Organization (WHO) declared to be a pandemic on the 11th of March 2020. This outbreak which has spread to all continents (Figure 1) has been characterized by exponential increase in infected cases, attributed deaths and socio-economic hardship. There are already more than 3.8 million confirmed cases globally with over 265 862 deaths (WHO Situation Report, 9 May 2020). The quality of the health system in a given country appears to not be an influential factor in preventing the installation or propagation of the pandemic as the nations with relatively robust health systems like in Europe and the USA have also been hit hard.

25. Improving Pandemic Preparedness: Lessons From COVID-19

The United States and the world were caught unprepared by the COVID-19 pandemic despite decades of warnings of the threat of global pandemics and years of international planning. The failure to adequately fund and execute these plans has exacted a heavy human and economic price. Hundreds of thousands of lives have already been lost, and the global economy is in the midst of a painful contraction. The crisis—the greatest international public health emergency in more than a century—is not over. It is not too early, however, to begin distilling lessons from this painful experience so that the United States and the world are better positioned to cope with potential future waves of the current pandemic and to avoid disaster when the next one strikes, which it surely will. This CFR-sponsored Independent Task Force report seeks to do just that, framing pandemic disease as a stark threat to global and national security that neither the United States nor the world can afford to ignore again. It argues that future pandemic threats are inevitable and possibly imminent; policymakers should prepare for them and identify what has gone wrong in the U.S. and multilateral response. One of the most important lessons of this pandemic is that preparation and early execution are essential for detecting, containing, and rapidly responding to and mitigating the spread of potentially dangerous emerging infectious diseases. As harmful as this coronavirus has been, a novel influenza could be even worse, transmitting even more easily, killing millions more people, and doing even more damage to societies and economies alike. This Task Force proposes a robust strategy consisting of critical institutional reforms and policy innovations to help the United States and the world perform better. Although there is no substitute for effective political leadership, The recommendations proposed here would if implemented place the nation and the world on a firmer footing to confront humanity’s next microbial foe. The Task Force presents its findings grouped into three sections: the inevitability of pandemics and the logic of preparedness; an assessment of the global response to COVID-19, including the performance of the World Health Organization (WHO), multilateral forums, and the main international legal agreement governing pandemic disease; and the performance of the United States, while also drawing lessons from other countries, including several whose outcomes contrast favorably with the U.S. experience.

Abstract:
When COVID-19 first emerged in late-2019 in Wuhan, China, without adequate containment, the virus and the economic shock waves that followed quickly spread across the world, leaving few countries unaffected by the contagion. COVID-19 quickly escalated into an ongoing and widespread global crisis, placing acute pressure on prevailing economic systems, governance structures, development institutions and health systems not experienced since the Spanish flu of 1918. In doing so, it crippled economies and ruptured trajectories of globalization and development, with a myriad of negative impacts as well as unintended positive effects such as reduced carbon emissions. As the pandemic continues to unfold, the responses of individual nation-states and sub-national regions have been both varied and divergent.

Abstract:
The outbreak of the novel coronavirus disease (COVID-19), caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), was first reported in Wuhan (Hubei province) China in December 2019. The virus has since then spread to all the 7 continents of the world, infecting over 14 million persons and killing over 600,000 [1] (as of July 19th, 2020.) By 30 January 2020, WHO Director General declared that the outbreak constitutes a Public Health Emergency of International Concern (PHEIC) [2]. The highest concentration of infected persons has shifted several times since mid-February 2020 from China to Iran, and then to Italy and Spain and is presently in the United States of America [3]. According to the Lancets, it is inevitable that Africa and Latin America will be experiencing the next wave of infections [4]. Since the introduction of the first case on February 14th, 2020, Africa as of July 19th, 2020, counted 683,905 infected persons and 14,684 deaths, thus contributing to 5% of the global caseload and to a little above 2% of the global death roll [5]. Although yet to be proven, several conspiracy theories have been proposed to explain these fewer cases observed in Africa [6]. Cameroon as of the 19th of July had recorded 16,157 cases with 373 deaths with 13,728 recovered cases [7]. This translates into a case fatality rate of 3.6% and a COVID 19 patient recovery rate of 85%. This ranks Cameroon as the 65th most affected country worldwide, the 7th in Africa [10] and the first [5] in Central Africa. The situation in Cameroon has pushed decision makers in Cameroon into taking a number of measures aimed at limiting the spread of the disease [8] in the country while at the time mitigating its impact on the economy [9]. Commendable efforts have been made by the government so far so such as the decentralisation of testing to 9 out of the 10 regions of the country (Rapport de Situation COVID 19 au Cameroon, Sitrep No 38). This report looks into COVID 19 situation in Cameroon with regards to the distribution of COVID 19 cases, the situation of health workers, some key mortality and morbidity indicators as well as the weekly evolution cases; making use of data from the beginning of the pandemic up to the 3rd of July 2020.

29. Flattening the Pandemic and Recession Curves

In the short run, the capacity of any country’s health system is finite (capacity of Intensive Care Units, number of hospital beds, number of skilled health professionals, ventilators….). This puts an upper bound on the number of patients that can be properly treated, at any given point in time and is represented by the flat line in the Figure. Unchecked, and given what we know of the transmission rate of the coronavirus, the pandemic would quickly overwhelm any health system, leaving many infected patients with deteriorating pulmonary conditions without any treatment. The fatality rate would surge. The threat is almost beyond comprehension. With a 2% case fatality rate baseline for overwhelmed health systems, and 50% of the world population infected, 1% of the world population -76 million people- would die. This scenario corresponds to the red curve in Figure 1. The part of the curve above the capacity of the health care system faces a sharply higher mortality risk (shaded red area).
