Abstract:
Every day, women across Latin America are terminating their pregnancies under illegal and unsafe circumstances. The number of unsafe abortions—which represent about 95 percent of the total—grew from 3.9 million in 2003 to 4.2 million in 2008, with the annual abortion rate holding steady during that period, at about 32 abortions per 1,000 women between the ages 15 to 44. That's more than double the rate in Western Europe (12 per 1,000)—where abortion is generally permitted without exception. The high rate of unsafe abortions, which is defined by the World Health Organization as a termination of pregnancy performed by individuals without necessary skills, or under conditions that are below minimum medical standards (or both), underlines the dismal state of reproductive rights in the region. And it has had chilling consequences for poor and rural women in particular.

The malaria parasite is thought to have killed more people throughout human history than any other single cause. Over the last decade, a large increase in resources for combating malaria — with the lion's share coming from U.S. taxpayers — has resulted in dramatic reductions in malaria cases and deaths in many countries. These successes spurred the Bill Melinda Gates Foundation and the World Health Organization (WHO) to call for global eradication of malaria in 2007.

Abstract:
The Global Polio Eradication Initiative (GPEI), a 26 year, $11 billion drive to eradicate poliovirus worldwide, is one of the largest public health initiatives ever. It is led by national governments together with the World Health Organization (WHO), the United Nations Children's Fund (UNICEF), the U.S. Centers for Disease Control and Prevention (CDC), Rotary International, and the Bill Melinda Gates Foundation. Involving a variety of partners from NGOs to universities and foundations and engaging millions of health workers and volunteers, the GPEI has provided billions of polio vaccine doses around the world. While it recently has faced new outbreaks and international spread of poliovirus, the GPEI has reduced the annual number of polio cases globally by more than 99 percent.

In 1990, the World Health Organization (WHO) started to downsize its renowned Global Programme on AIDS, despite continued donor and member state support. This turnaround has decisively contributed to WHO's loss of leadership in HIV/AIDS politics. From the viewpoint of both rationalist and constructivist theories of international organisation (IO) agency, an IO engaging in 'mission shrink' is a striking irregularity. In order to account for such apparently self-defeating behaviour, this article adopts an open systems view of IOs and identifies trans-organisational coalitions as important agents of IO change. I argue that subunit dynamics rather than systemic conditions drive IO behaviour, in particular where member states' material power and their formal control of organisational veto positions do not coincide. This approach will be used to retrace the changes in subunit coalitions that drove WHO's erratic HIV/AIDS programme and thus to solve this puzzle of 'mission shrink'. On the basis of insights from the WHO case, the article concludes by offering a heuristic of trans-organisational coalitions and the types of IO change associated with them.

According to hitherto available data, health expenditures in Pakistan are relatively low in international comparison. Data2 published by the World Health Organization (WHO) for the year 2005 shows a lack of Pakistani health expenditures in most indicators, compared to other low-income countries (LIC). To answer the question whether these results reflect the real situation in Pakistan or whether they exist due to statistical problems, Pakistan, for the first time, developed its National Health Accounts (NHA) in 2009. Only the availability of good estimates of health expenditures allows for evidence-based policymaking and therefore for good governance.

There is limited international structure to manage the ever important phenomenon of human migration and its associated challenges. This is particularly true with respect to the international migration of health workers, where bilateral agreements between sending and receiving nations have been repeatedly and urgently called for in the context of a global health workforce crisis. There remains, however, significant lack of clarity on the precise role, form, and content bilateral agreements should take to serve a health-related purpose. This Guidebook, including presentation of two model bilateral agreements, aims to provide guidance to further international cooperation around the critical and highly sensitive area of health worker migration.

The fear of pandemics, the significance of health in the fight against poverty, and a proliferation of new actors have increasingly directed international attention towards global health. In this context, the financial resources available for global health as well as the flexibility in problem solving have increased significantly, but at the cost of a proliferation of public and private actors which tends to inhibit the effective use of these resources. The Paris Declaration on Aid Effectiveness has been broadly embraced in global health, and there has been a reappraisal of the coordinating role of the World Health Organization (WHO).

Abstract:
Globalization of infectious disease transmission has led to international and regional initiatives to improve surveillance and response. The World Health Organization's revised International Health Regulations provide a more robust legal framework for outbreak investigations. New regional networks are strengthening collaborative approaches to prevention of pandemics even in parts of the world where political tensions usually run high. To fulfill the promise of these new mechanisms, the United States should integrate capacity development for disease surveillance into its global health strategy, including providing greater investment in laboratories, training and technical assistance for low-income countries. Effective international cooperation has not extended to creating a system for equitable distribution of vaccines, resulting in vast disparities in availability of vaccines between richer and poorer countries. As a result, political tensions between wealthy and low-income countries have increased. Unless inequity is addressed, global health security will not advance.
