1641. Antiretroviral Therapy Awareness and Risky Sexual Behaviors: Evidence from Mozambique

Abstract:
This paper studies the effect of increased access to antiretroviral therapy on risky sexual behavior, using data collected in Mozambique in 2007 and 2008. The survey sampled both households of randomly selected HIV-positive individuals and households from the general population. Controlling for unobserved individual characteristics, the findings support the hypothesis of disinhibition behaviors, whereby risky sexual behaviors increase in response to the perceived changes in risk associated with increased access to antiretroviral therapy. Furthermore, men and women respond differently to the perceived changes in risk. In particular, risky behaviors increase for men who believe, wrongly, that AIDS can be cured, while risky behaviors increase for women who believe, correctly, that antiretroviral therapy can treat AIDS but cannot cure it. The findings suggest that scaling up access to antiretroviral therapy without prevention programs may not be optimal if the objective is to contain the disease, since people would adjust their sexual behavior in response to the perceived changes in risk. Therefore, prevention programs need to include educational messages about antiretroviral therapy and address the changing beliefs about HIV in the era of increasing antiretroviral therapy availability.

1642. Food Crisis, Household Welfare, and HIV/AIDS Treatment: Evidence from Mozambique

Abstract:
Using panel data from Mozambique collected in 2007 and 2008, the authors explore the impact of the food crisis on welfare of households living with HIV/AIDS. The analysis finds that there has been a real deterioration of welfare in terms of income, food consumption, and nutritional status in Mozambique between 2007 and 2008, among both HIV and comparison households. However, HIV households have not suffered more from the crisis than others. Results on the evolution of labor-force participation suggests that initiation of treatment and better services in health facilities have counterbalanced the effect of the crisis by improving the health of patients and their labor-force participation. In addition, the authors look at the effect of the change in welfare on the frequency of visits to health facilities and on treatment outcomes. Both variables can proxy for adherence to treatment. This is a particularly crucial issue as it affects both the health of the patient and public health because sub-optimal adherence leads to the development of resistant forms of the virus. The authors find no effect of the change in welfare on the frequency of visits, but they do find that people who experienced a negative income shock also experienced a reduction or a slower progress in treatment outcomes.

Abstract:
The Palestinian statehood-by-2011 program, framed through neoliberal institution building, redefines and diverts the Palestinian liberation struggle. Focusing on its economic aspects, and in particular the underlying neoliberal thought that goes beyond narrow economic policy applications, this essay argues that the program cannot succeed either as the midwife of independence or as a strategy for Palestinian economic development. Its weaknesses, the authors contend, derive not only from neoliberalism's inability to deliver sustainable and equitable economic growth worldwide, but also because neoliberal “governance” under occupation, however “good,” cannot substitute for the broader struggle for national rights nor ensure the Palestinian right to development.

Abstract:
The political stalemate in Côte d'Ivoire following the November 28, 2010, presidential election continues. The majority of the international community recognizes Alassane Ouattara as the winner, but Laurent Gbagbo, the sitting president, insists he won. Financial and diplomatic sanctions imposed on the Gbagbo administration have thus far not forced Gbagbo from power. Maintaining international pressure and focus is critical to resolving the Ivorian crisis, but African states are increasingly divided on how to proceed. The power-sharing arrangement settled on by five African nations in recent elections sets a dangerous precedent. Losers with a strong militia may find it easier to use threats of violence or actual violence to retain a critical power role, thus subverting the intent of the election. African states will continue to experience violence during elections until the security sector is reformed, states refrain from holding elections while militias remain mobilized and armed, elections can be clearly and independently verified, institutions are politically independent, and policies exist to discourage the violent acquisition of power.

But Africa also has three major opportunities that can help transform its agriculture to be a force for economic growth. Coverage of the National Health Insurance Scheme (NHIS) has been hugely exaggerated, and could be as low as 18% Every Ghanaian citizen pays for the NHIS through VAT, but as many as 82% remain excluded Twice as many rich people are signed up to the NHIS as poor people. 64% of the rich are registered compared with just 29% of the poorest Those excluded from the NHIS still pay user fees in the cash and carry system. Twenty five years after fees for health were introduced by the World Bank, they are still excluding millions of citizens from the health care they need An estimated 36% of health spending is wasted due to inefficiencies and poor investment. Moving away from a health insurance administration alone could save US$83 million each year. Enough to pay for 23,000 more nurses Through savings, good quality aid but primarily improved progressive taxation of Ghana's own resources, especially oil, the government could afford to increase spending on health by 200%, to US$54 per capita, by 2015 This would mean the government could deliver on its own promise to make health care free for all – not just the lucky few at the expense of the many.

Abstract:
It is one of the sad facts of recent human history that the economic prosperity enjoyed by so many remains unknown to most of the rest. The causes of poverty have long been debated and much has been spent in the effort to ameliorate it. Reliable estimates suggest as much as $2.3 trillion has been spent over the last several decades, most of it in the form of sponsored aid programs conceived and pursued by governments and large foundations in developed countries. Despite this investment, however, poverty remains widespread and has worsened in many places, especially in Africa. These basic facts now fuel a vigorous debate over the scale and ultimate value of traditional aid programs and a search for more effective solutions.
